Which case is your code stress failing in ?
If it is SUS case than it could be because of the supports used.Check if the span is correct and the extent of sagging.Furthermore check your input and still if you cant find any problem than recalculate the schedule of the pipe.
If your pipe is failing in EXP case than view it in 3D and see where it is failing and what stresses are causing the failure. Than proceed with your best judgment.

From the word CODE STRESS FAILURE. It is a limitation set by the code. to defined your problem, read your refference code. This is quite big discussion, only i will advice you to use the search function and search for Code compliance. If your using the B31.3 code read, chapter II.
