I have received an unconfirmed report of a boiler failure where an alleged P91 tube failed after less than 6 weeks of operation.

A brief quote from my unconfirmed report, "investigation indicates that the pipe was actually manufactured by a pipe mill, then sold to another 'pipe producer' who allegedly did some type of heat treatment to it then sold it to a third 'pipe company' who polished the OD and sized the ends... This last company then certified it as 'Made in the USA' and shipped it to the fabrication company."

This chain of events should cause a chill to run up and down each and every one of your spines. Anyone who has read the paper I posted earlier on the subject of the proper handling of P91 should immediately see several things in the above chain of events that scare the living daylights out of him or her.

Any one of you who has ANY involvement in the design, specification, or installation of P91 piping needs to be very sure that you KNOW what happened to that pipe at EVERY stage of the fabrication process. Supposedly, two people are dead and others are still in critical condition.

Any P91 material that you are not ABSOLUTELY SURE of should be subjected to examination when it is received on the job site. Make sure that the metallurgist doing the examination has experience with P91!

I will post more when I get confirmation of the details.

Regretably, the people who paid the highest price for this "lesson learned" will not be able to benefit from it.

The B@#$$@%d's that were involved should be held criminally negligent given a fair trial and then be executed....

Internal Pressure is an unrelenting load, and unforgiving load. Either the pipe can withstand it or it fails no ifs and or buts......
